TCA CROSS: Precision for Ice-Pick Scars

TCA CROSS (Chemical Reconstruction of Skin Scars) involves the precise application of high-concentration trichloroacetic acid (70–100%) into the base of individual ice-pick scars using a fine-tipped applicator. The controlled chemical burn triggers a focused inflammatory response that stimulates collagen synthesis at depth, literally filling the scar from below.

The procedure is highly precise—targeting each scar individually—and requires minimal downtime. Multiple sessions are typically required, spaced 6–8 weeks apart. Results improve progressively as new collagen remodels the treated channels. TCA CROSS is specifically indicated for narrow, deep scars; applying it to rolling or boxcar scars would be clinically inappropriate and potentially harmful.

Subcision: Releasing Tethered Rolling Scars

Rolling scars are caused by fibrous bands of tissue that anchor the skin surface downward to deeper structures. No amount of topical treatment, laser, or surface resurfacing will correct this—because the problem is structural, not superficial. Subcision addresses this directly.

Using a specialized tri-beveled needle inserted beneath the scar, Dr. Rashmika Reddy mechanically severs these fibrous tethers. The released skin lifts, and the trauma of subcision triggers collagen deposition in the newly created space. A single subcision session can produce dramatic improvement in the undulating appearance of rolling scars.
