Ideal Candidates
Specifically for patients with rolling acne scars — the wavy, undulating scar type caused by fibrous bands anchoring the skin surface down to the subcutaneous tissue. Subcision is the most effective single procedure for rolling scars. Not effective for ice-pick or boxcar scars, which require TCA CROSS and MNRF respectively.

Medical Foundation
The Science Behind The Treatment
The area to be treated is mapped and marked under bright lighting or dermoscopy. Topical anaesthetic cream is applied for 45 minutes and local anaesthetic may be injected subcutaneously for deeper comfort. A sterile 23G needle (or Nokor needle) is inserted through a single puncture point at the scar margin, bevel up, at a subdermal depth parallel to the skin surface. The needle is swept in a fan-like motion beneath the scar to cut the fibrous bands tethering the skin downward. A popping sensation may be felt when each tether is severed. Multiple passes may be needed per scar. Post-subcision, the treated areas appear elevated and bruised — this haematoma is intentional and beneficial. Filler or GFC may be injected immediately after. The bruising resolves over 7–10 days, during which the collagen framework organises. Patients see progressive improvement over 4–6 weeks.

Pre-Treatment Steps
- • Avoid active retinoids and harsh exfoliants 3 days prior
- • Inform doctor of any active cold sores or medication
- • Arrive with clean, makeup-free skin
Post-Treatment Guidelines
- • Bruising at the treated sites is expected and can last 7–10 days — this is normal and indicates the haematoma is forming the collagen scaffold.
- • Do not massage or press on the subcised areas for 1 week — the haematoma structure must be preserved.
- • Apply prescribed arnica gel to reduce bruising discoloration if desired.
- • Avoid makeup for 24 hours over the subcised areas.
- • Sleep on your back (face up) for the first 3 nights to prevent pressure on the treated sites.
- • Restart prescribed skincare products after 48 hours.
- • Avoid alcohol for 5 days — alcohol delays haematoma resolution and affects collagen organisation.
