Ideal Candidates
For patients with sebaceous cysts (epidermoid cysts, pilar cysts), lipomas, or other benign subcutaneous lumps requiring surgical removal. The procedure is appropriate for cysts that are asymptomatic, growing, or becoming recurrently inflamed. Infected cysts require antibiotic treatment and a 4–6-week waiting period before elective excision.

Medical Foundation
The Science Behind The Treatment
The cyst is assessed for size, location, and relationship to surrounding structures. The area is sterilised with betadine and draped with sterile fenestrated draping. Local anaesthetic (1% lignocaine with 1:100,000 adrenaline) is injected around and beneath the cyst. A fusiform (elliptical) incision is made over the cyst, including the central punctum (keratin opening). Sharp dissection with iris scissors carefully separates the cyst capsule from the surrounding subcutaneous tissue while keeping the capsule intact. The entire cyst with intact capsule is delivered through the incision. Haemostasis is achieved with the electrosurgery unit. The wound is closed in layers: interrupted 3-0 Vicryl sutures to close the dead space, followed by interrupted 3-0 Prolene skin sutures. A sterile wound dressing is applied. Sutures are removed at 7–14 days depending on location.

Pre & Post Procedure Guidelines
Pre-Treatment Steps
- • Avoid active retinoids and harsh exfoliants 3 days prior
- • Inform doctor of any active cold sores or medication
- • Arrive with clean, makeup-free skin
Post-Treatment Guidelines
- • Keep the wound dressing dry and clean for 48 hours.
- • Change dressing as instructed (usually day 2, then every 2–3 days until suture removal).
- • Apply prescribed antibiotic cream to the wound edges at every dressing change.
- • Avoid strenuous activity and heavy lifting for 1 week to prevent wound dehiscence.
- • Return for suture removal at the scheduled appointment — do not attempt to remove sutures at home.
- • After suture removal, apply silicone gel or silicone tape to the scar for 3 months to optimise scar outcome.
- • If redness, swelling, discharge, or fever develops, return to the clinic immediately — these are signs of wound infection.
