Ideal Candidates
For patients with keloids (scars that grow beyond the original wound boundary) or hypertrophic scars (raised scars within the wound boundary). Common locations: earlobes (post-piercing), chest, shoulders, back, and jaw. More common in people of African and Indian descent (Fitzpatrick III–V). Also appropriate for hypertrophic surgical scars and caesarean section scars.

Medical Foundation
The Science Behind The Treatment
The keloid or hypertrophic scar is assessed for dimensions (height, length, width), symptom score, and previous treatment history. Topical anaesthetic (EMLA) is applied 30 minutes prior. Triamcinolone acetonide (10–40mg/ml depending on scar site and severity) is drawn into a syringe with a 26G needle. The needle is inserted directly into the body of the scar (not beneath it) at multiple points, distributing the corticosteroid throughout the lesion. The scar will blanch during injection. Patients experience moderate pressure and mild burning during the injection — this improves with subsequent sessions as the scar softens. The patient is prescribed silicone tape for daily application at home between sessions.

Pre-Treatment Steps
- • Avoid active retinoids and harsh exfoliants 3 days prior
- • Inform doctor of any active cold sores or medication
- • Arrive with clean, makeup-free skin
Post-Treatment Guidelines
- • Apply prescribed silicone gel or silicone tape to the scar daily between sessions.
- • If the scar is in a pressure area (earlobe), wear prescribed pressure earring or clip.
- • Avoid sun exposure on the scar — UV stimulates melanocyte activity in scar tissue causing permanent hyperpigmentation.
- • Apply SPF 50+ to all scar areas exposed to the sun.
- • Do not re-pierce the ear or traumatise the area during keloid therapy.
- • Take prescribed vitamin E oil for massage on the scar between sessions.
- • Inform the clinic if the scar becomes very red, itchy, or grows between sessions — this suggests the keloid is actively growing and may require more frequent injection.
